LaFayette Manor, Inc. complies with applicable state anti-discrimination laws and Federal civil rights laws and does not discriminate based upon race, color, national origin, handicap or disability, familial status, religious creed, ancestry, age, sex (consistent with the scope of sex discrimination described at 45 CFR 92.101(a)(2), ability to pay, use of guide or support animals because of the blindness, deafness or physical handicap of the resident or because the resident is a handler or trainer of support or guide animals.

LaFayette Manor, Inc. does not exclude people or treat them differently because of race, color, familial status, religious creed, ancestry, age, sex (consistent with the scope of sex discrimination described at 45 CFR 92.101(a)(2), national origin, ability to pay, handicap or disability, use of guide or support animals because of the blindness, deafness or physical handicap of the resident or because the resident is a handler or trainer of support or guide animals. No resident will be excluded from participation in, be denied benefits of, or otherwise be subjected to discrimination in the provision of any care or services whether by the Facility directly or through a contractor or any other entity with which the facility arranges to carry out its programs, services and activities. This applies not only to care and services, but also to room assignments, staff assignments, granting of privileges to medical staff, room transfers, and to use of the facility’s physical space.

In addition, LaFayette Manor, Inc.:

  • Provides people with disabilities reasonable modifications and free appropriate auxiliary aids and services to communicate effectively with us, such as:
    • Qualified sign language interpreters.
    • Written information in other formats (large print, audio, accessible electronic formats, other formats).
  • Provides free language services to people whose primary language is not English, such as:
    • Qualified interpreters.
    • Information written in other languages.
  • If you need these services, contact Samantha Fike, BSW.

If you believe that LaFayette Manor, Inc. has failed to provide these services or discriminated in another way on the basis of race, color, familial status, religious creed, ancestry, age, sex, gender, sexual orientation, gender identity or expression, national origin, ability to pay, handicap or disability, use of guide or support animals because of the blindness, deafness or physical handicap of the resident or because the resident is a handler or trainer of support or guide animals, you can file a grievance with:
